Care_LINK

2025/2026 Venture   |   Based in: Tanzania   |   Impact region: Tanzania

The business at a glance

In Tanzania, 7 out of 10 people living with chronic illnesses - like diabetes or hypertension - miss their clinic follow-ups. This is largely often due to long travel distances, overcrowded clinics, and the cost of taking time off work. With no remote monitoring system in place, many experience preventable complications such as stroke, hypertensive heart disease, retinopathy - and tragically, even death.

Care_LINK is a health tech social enterprise transforming chronic illness care through a mobile platform that enables patients to log vital signs from home, receive real-time alerts for health risks, and stay connected to their doctors between visits. The model is designed for chronically ill patients in middle-income and underserved communities. The business generates revenue through subscription tiers and consultation fees, and profits support REIZA Health Care Initiative, their connected non-profit providing life-saving surgeries for families in poverty.

Care_LINK has built a prototype app and piloted it with a few communities, and now, with our support, they’ll refine their business strategy and prepare for launch.
